Terms of Service

Allay Life provides genetic counseling and consulting services, consistent with a genetic counselor’s scope of practice as defined by the National Society of Genetic Counselors (NSGC) and the American Board of Genetic Counseling (ABGC) and does not provide medical advice. The content provided on this website and through any services provided by Allay Life is not intended for medical diagnosis or treatment.  Seek the advice of your physician or a qualified healthcare professional with any questions you may have regarding any medical condition.

Payment & Cancellation Policy

Payment is required to secure your 1-on-1 session. There is no charge to reschedule or cancel a session if done 48 hours or more in advance of your scheduled session.  Sessions cancelled within 48 hours of the scheduled time will be charged the full fee.

Insurance

We are a private pay service only.  You may be able to use your Health Savings Account (HSA) to pay for genetic counseling services. We do not bill insurance. 

If requested, we can provide you with a receipt for you to submit to your insurance provider for possible reimbursement. We cannot guarantee whether our services will be reimbursed by your insurance provider. If you elect to submit a receipt of services to your insurance provider for reimbursement, Allay Life may be contacted by your insurer to supply additional information to them for consideration of reimbursement. By using this site, you consent to allowing us to provide such information without notice to you. We will, however, consider any requests from you to restrict our disclosure of your information. 

Information Collection, Use and Sharing

We are the sole owners of the information collected on this site. We only have access to collect information that you voluntarily give us via email or other direct contact from you. We will not sell or rent this information to anyone. We will use your information to respond to you, regarding the reason you contacted us. Except as set forth herein (e.g., “Insurance”), we will not share your information with any third-party outside of our organization without your permission. 

Security

We take precautions to protect your information. We have established and maintain physical, electronic and procedural safeguards to protect your information. 

Privacy Policies

At Allay Life, your privacy is important, and we are committed to protecting your personal information. We do not collect your information through this website. We use third-party websites to collect your information. See “Links to Other Sites” below. 

Profi is our third-party scheduling system within which names and credit card information are stored. You agree to be subject to  Profi’s Privacy Policy found here. Stripe is our third-party credit card processing service. You agree to be subject to Stripe’s Privacy Policy found here. 

You understand and accept that despite reasonable security measures, the security of your information can never be completely guaranteed. In no event shall Allay Life be responsible for the privacy and security of information that is sent through these third-party service providers. 

Accurate Self-Representation

While using our website or third-party websites, you agree: 

• Not to misrepresent yourself, your state of residence and/or the place of service of your 1-on-1 appointment.

• Not to share any confidential information about a person other than yourself unless you are authorized to do so, including, but not limited to, copies of relatives’ genetic testing reports and other medical records.

• To provide accurate reporting of personal and family medical history, including results, to the best of your ability. 

If you learn of new or alternate information after the time of your 1-on-1 session, you are encouraged to share this information and a follow-up session may be recommended for a fee.

This Site may contain third party-owned content and may also include hypertext links to third party-owned websites. We provide such third-party content and links as a courtesy to our users. We have no control over any third party-owned websites or content referenced, accessed by, or available on this Site and, therefore, we do not endorse, sponsor, recommend, or otherwise accept any responsibility for such third-party websites or content or for the availability of such websites. We do not accept any liability arising out of any allegation that any third party-owned content (whether published on this, or any other website) infringes the intellectual property rights of any person or any liability arising out of any information or opinion contained on such third-party website or content. Miscellaneous

These terms comprise the entire agreement between you and Allay Life with respect to your use of the site and supersedes all prior agreements between the parties regarding the subject matter contained herein.  These terms shall be governed by, and construed and enforced in accordance with, the laws of the State of Iowa, without regard to the choice of law provisions of that State. Any claim relating to these terms must be brought within one year of the date on which the claim arises. Any dispute between the parties relating to these terms shall be resolved only in the Iowa District Court for Linn County or the United States District Court for the Northern District of Iowa (Cedar Rapids Division).  In the event that any provision of these terms becomes or is declared by a court of competent jurisdiction to be illegal, unenforceable or void, these terms shall continue in full force and effect without said provision.  You agree that money damages would not be a sufficient remedy for breach of these terms and Allay Life shall be entitled to specific performance for any such breach in addition to any other remedies available to it at law or in equity. No waiver of any term in these terms shall be deemed a further or continuing waiver of such term or any other term.
